For your convenience we have attached the Boys & Girls Club Application.  The forms may be turned in to the SHPS Office or at the main clubhouse located at 1420 Tainter St. in St. Helena or emailed to marisa@bgcshc.org - PAYMENT must be made directly to the club please.
